Quantic PMI Model PEC-30-0R118-5R0-20-12-SFF is a 0.1 to 18 GHz low noise amplifier that offers a gain of 25 to 32 dB and a gain flatness of ±1 dB with a noise figure of 5 dB @ 18 GHz.  The unit is supplied in a standard PE2 housing that can be used as a SMA connectorized or a surface mount component.
