Quantic PMI Model PEAFS3-14-0R2535R0-6R5-23-12-292MF is an 8 to 18 GHz low noise amplifier that provides over 13 dB gain with a gain flatness of ±1 dB maximum; noise figure of 4 dB; OP1dB of 24 dB minimum; VSWR IN/OUT of 2.0:1; and SMA connectors.   This amplifier is supplied in housing that is 0.53" x 0.70" x 0.26" and can be used as a connectorized or a surface mount component.
